5/30/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Anthony appeared to be in a content mood evident by his facial expressions such as a smile on the face. QMHP-C engaged Anthony as well as his mother in a discussion to determine behaviors since the last session. Mother reports he continues to have issues with respecting those who are in authority. 2. Behavior: Anthony appeared to be cooperative throughout the session but needing redirection from QMHP-C due to him leaving the room and coming back after hitting his siblings. 1.Intervention: QMHP-C reviewed with Anthony the behavioral expectations he has had problems with in the past in the home school while pointing out to him the similarities in conflict being his defensive attitude. QMHP-C used conversation to coach Anthony into positively and openly express his feelings. QMHP-C listened intently to Anthony describe his most recent interactions with family and friends, referring to his on-going feud with his sister. 2. Intervention: QMHP-C prompted Anthony with a series of open leading questions to prompt open and honest expression of feelings while encouraging him to seek out solutions to his problem. QMHP-C assisted Anthony reviewed alternative coping strategies that best work to help him decrease negative behavioral displays including walking away from conflict and considering his surroundings before acting out and the ramifications of his behaviors in that setting. QMHP-C encouraged Anthony to utilize effective communication strategies rather rely on his traditional impulsive tactics to resolve conflict. 1.Response: Anthony stated no changes needed to be made to safety plan. Anthony initially presents an unbothered attitude until he is fully engaged in conversation. Anthony is able to be redirected by QMHP-C through discussion that challenged Anthony to share his thoughts on family issues. 2. Response: Anthony never admits to having a poor attitude and demeanor choosing rather to blame others around him for his mischief and lack of focus on positive behavioral tendencies.
